After you have received a request to submit one of the forms below, please download it to your computer and save it unless the form must be printed out (e.g. the verification forms). After you have saved the form, you can open the document to complete it and sign the form with your electronic signature. If your form must be submitted in person, please stop by our office on the 4th floor of the Briscoe Library otherwise you can email the form (using your livemail account) to vsfa@uthscsa.edu.
- 2025-26 State Aid Worksheet – required of any student receiving assistance from state-funded programs (e.g. Texas Public Education Grants, Designated Scholarships, or CAL loans for example). Please submit this worksheet only after receiving a request from our office.
- 2025-26 Waiver or Exemption Worksheet -required of any student receiving a state waiver or exemptions.
- Texas Firefighter Tuition Exemption Request – For use in term(s) fall 2025, spring and summer 2026 (includes the 2025-26 State Aid Worksheet required for state-funded assistance).
- Pell Grant Statement of Eligibility – Please submit this statement, after receiving a request from our office, confirming you are not receiving a Pell grant at another school and that you are still working on your first bachelors degree.
- TEXAS Grant Statement of Eligibility – Incoming and continuing students must complete this form to be considered for a TEXAS grant. UT Health San Antonio only receives “renewal” funds for prior recipients and cannot offer initial awards. Click here for more details.
- Independent Verification Worksheet 2025-26 – This form is used if you are an independent student selected for verification. You must print the form and complete the requested information. Please do not submit this form unless requested.
- Dependent Verification Worksheet 2025-26 – This form is used if you are a dependent student selected for verification. You must print the form and complete the requested information. Please do not submit this form unless requested.
- Identity and Statement of Educational Purpose 2025-26 – You must print the form and complete the requested information. To order a tax return transcript click here or call 1-800-908-9946 if you are asked to provide this information.
- Verification of Non-Filing – To order an IRS-issued Verification of Non-Filing, click here to access the IRS Get Transcript Online website. Please note the the IRS now uses a two-step multi-factor authentication process that requires you to register before submitting an online transcript request. If you are not able to successfully register, you can still request it via mail using IRS Form 4506-T. Please do not submit this form unless requested.
- Texas Application for State Financial Aid 2025-26 – This form is used if you are a resident of the State of Texas and you do not meet the citizenship requirements for Federal student Aid. Please submit this form along with the TASFA verification worksheet, 2023 tax transcript, and your 2023 W2 form(s). If you are a dependent student we will need your parents’ income information too. Please contact our office if you are not sure of your dependency status. You must print the form and complete the requested information.
- TASFA Verification Worksheet 2025-2026 – Complete and submit this form along with your 2025-26 Texas Application for State Financial Aid (TASFA) if you do not meet the citizenship requirements for Federal Student Aid but are classified as a Texas Resident by the Office of the University Registrar. You must print the form and complete the requested information.
- Cost of Attendance Adjustment Request 2025-26 – This form should be submitted if you would like to be considered for a Cost of Attendance increase. Examples of allowable increases include dependent care expenses, vehicle repairs, or other educational costs not included in your current Cost of Attendance. Medical and dental expenses are not part of a student’s cost of attendance and are treated differently according to Title IV regulations. Please contact a financial aid counselor for more details.
- Nonresident Teaching\Research Assistant Waiver Form and 2025-26 Waiver\Exemption Worksheet *** Download this form using Mozilla’s Firefox to use the electronic signature feature *** Waiver requests are submitted annually, certifying the student will be employed at least one-half time by UT Health San Antonio in a Teaching Assistant (TA) or Research Assistant (RA) position. College work study students are not eligible to receive this waiver based solely on their employment as a student worker. Departments will need to submit proof of student’s appointment as a TA or RA, using the Human Capital Management (HCM) system, and waivers will be applied to each semester that falls within the position start and end dates after confirming the student’s enrollment status is at least half time for the term the waiver will be applied. Students in the Graduate School of Biomedical Sciences must be enrolled full-time to qualify for the waiver in accordance with their policy.
