The forms below must be completed in order for our office to receive clearance for you to begin working. Please carefully review each form you complete and bring them to our office on the 4th floor of the John Briscoe library along with your identifying documents (drivers license, social security card and\or US passport),
- College Work Study Student Contract – read and sign
- Criminal Background Check authorization – please remember to sign page 2
- I-9 FORM – only the first page needs to be printed and signed
- W-4 Form – only the first page needs to be printed and signed
- Direct Deposit Form – please remember to sign the completed form
- I.D. and Social Security Card OR U.S. Passport – we will make copies for you
After you submit your paperwork, please allow time for the forms to be processed and your background check to be completed. Our office will send an email to your school livemail account with your clearance date. Please send the email to your supervisor so they know you are eligible to begin working.
