- Texas Firefighter Tuition Exemption Request – For terms Fall 2022, Spring 2023, and Summer 2023.
- Child Support Statement – Self-certification form for students receiving state financial assistance, indicating they are not behind on child support payments, as required by Texas Family Code, Title 5, Section 231.006. This form must be submitted every academic year.
- TEXAS Grant Statement of Eligibility – Incoming and continuing students must complete this form for consideration. UT Health San Antonio does not receive an initial allocation for this program and can only consider students who have previously received a grant. Click here for more details.
- FAFSA – Submit a Free Application for Federal Student Aid online here. Review the steps after you submit your application here. Please consider which award year you are applying for:
- 2021 – 2022 applies to fall 2021, spring 2022, and summer 2022
- 2022 – 2023 applies to fall 2022, spring 2023, and summer 2023
- IRS Data Retrieval Tool Instructions – The best way to report income in the FAFSA is by using the IRS Data Retrieval Tool (IRS DRT) within the application.
- Independent Verification Worksheet 2022-2023 – This form is used if you are an independent student selected for verification. Please do not submit this form unless requested. It can prolong your application process.

- Verification of Non-Filing – To order an IRS-issued Verification of Non-Filing, click here to access the IRS Get Transcript Online website. Please note the the IRS now uses a two-step multi-factor authentication process that requires you to register before submitting an online transcript request. If you are not able to successfully register, you can still request it via mail using IRS Form 4506-T. Please do not submit this form unless requested. It can prolong your application process.
- Texas Application for State Financial Aid 2022-2023
This form is used if you are a resident of the State of Texas and you do not meet the citizenship requirements for Federal student Aid. Please submit this form along with the TASFA verification worksheet, 2020 tax transcript, 2020 W2 form(s), and student’s proof of Selective Service Registration (for males only). If you are a dependent student we will need all of your parents’ income information too. Please contact our office if you are not sure of your dependency status.
