Transcript Requests

Per guidelines in accordance with the CDC and the University of Texas Health Science Center, the Office of the University Registrar is closed at this time to protect our students, faculty and staff from the spread of COVID-19 (coronavirus).

Transcripts may still be requested, but requests for paper copies will only be processed one day during the business week. Requests for electronic transcripts will be processed on a daily basis, Monday-Friday. Before requesting an electronic transcript, the requestor should verify that the recipient will accept an electronic PDF transcript.


**Official transcripts cannot be released unless the student (or alumni) has satisfied all financial or other obligations to the University.**

Former students should submit official and unofficial transcript requests through the National Student Clearinghouse.

Current students should submit official transcript requests through the National Student Clearinghouse, but may print unofficial transcripts through My Student Center.

Requesting transcripts through the National Student Clearinghouse:

Go to the National Student Clearinghouse (NSC) Online Services. Please note our institution is listed as “University of Texas HSC-San Antonio” in the NSC system.

  • Fee: $10 per transcript and $2.50 processing fee per request (credit card only). Electronic transcripts incur an additional $1.75 fee. Credit cards are charged after the transcript is sent.
  • Time Frame: The service is available 24 hours a day, 7 days a week. After being ordered through the NSC, transcripts are then processed by the Health Science Center Office of the University Registrar. Orders placed after regular working hours are processed within 1-2 business days, providing the requester has no outstanding holds. Online transcript requests submitted while the UT Health Science Center is closed for holidays will be processed in the order in which they were received when the UT Health Science Center, Office of the University Registrar reopens.
  • Pick-Up Service:  Transcripts will be available after 10 a.m., 1-2 business days later.  Official identification card (School ID/driver license) must be presented when picking up transcript(s).
  • Location: Central Campus, School of Medicine Bldg. , Room 317L, View Map
  • Hours of Operation: Monday-Friday, 8:00 a.m. – 5:00 p.m., Closed Holidays
  • Instructions: The UT Health Science Center San Antonio has authorized the National Student Clearinghouse (NSC) to provide online transcript ordering service to current and former students for an additional fee.

You can order as many transcripts as you need in a single session. To ensure confidentiality and to comply with federal regulations governing educational records, you must authenticate yourself. You will be presented with the Consent to Release form. You then have two options.

  1. Sign the consent form using your mouse as a virtual pen.
  2. Print, sign, and return the document to the NSC via mail, fax, or by scanning the document and forwarding by e-mail. The NSC  address, fax, and e-mail information are located on the consent form itself. Do not return the consent form to the UT Health Science Center. In addition to the convenience of credit card payment, the online service provides e-mail notifications once orders are received and processed by the UT Health Science Center Office of the University Registrar.

Delivery Options


Official transcripts can be mailed via First Class USPS, unless otherwise specified. Transcripts may be sent via expedited mail for an additional fee.


Official transcripts can be sent electronically in a PDF format. It is your responsibility to ensure that the recipient accepts electronic PDF transcripts. The PDF option must be selected when ordering.

NOTE: Recipients and their email address must be designated when placing the order through the NSC secure website. Recipients will receive two separate emails that include login information and password. Forwarded copies are classified as unofficial transcripts. Recipients have 30 days from the first day the transcript is opened or accessed to view, print, and save the PDF transcript. After 30 days the link which allows access to the transcript will expire.

Hold for Pick-Up

A transcript order may be designated as “Hold for Pick-Up.” Official transcripts may be picked up at the Office of the University Registrar located at 7703 Floyd Curl Drive, Medical School, 3rd Floor, Room 317L within 1-2 business days. Our office hours are 8:00 AM – 5:00 PM, Monday through Friday. You will receive an email notification when your transcript is ready for pick-up. Photo identification is required for students, parents and other third parties to pick-up official transcripts.

Note: Transcript(s) pick-up from another individual than the student: At the time of the transcript order, a written authorization from the student is required. There is a comment field on the transcript order request that allows for the designation of a third party pickup. Sample authorization statement: I, (name of student), authorize (name of third party) to pick up my transcript on my behalf. Include the student signature and date.

Transcript Fees

Transcripts are $12.50 per copy. Transcripts may be sent via expedited mail for an additional fee of $12.50.

Status of Transcript Requests

Only current UTHSCSA students may request unofficial transcripts. Get an unofficial transcript through My Student Center.
