Hazlewood Exemption

Hazlewood Benefits:

The Hazlewood Exemption provides an education benefit to honorably discharged or separated Texas veterans and to eligible dependent children and spouses of Texas veterans.

 

Please review the types of programs below to determine your eligibility criteria and the forms you will need to submit to receive the exemption.

 

Eligible veterans, their children and spouses may receive an exemption from the payment of all tuition, and certain fees for up to 150 semester credit hours, provided they are enrolled in state-funded courses.  While most fees are covered, the list below shows fees that are not covered by the exemption:

  • Malpractice fee
  • Student health insurance
  • Student services fee
  • Medical services fee
  • Fitness center fee
  • Academic advising fee
  • BSN to DNP program fee

Additional Hazlewood Information:

  • Complete Applications and all supporting documentation must be received by UT Health San Antonio no later than the last day of class in order to be evaluated for the semester or term.  Medical and Dental Students must submit all required documents the last day of Fall classes for the academic year.
  • This does not allow for retroactive application of benefits to an earlier term
  • Notification from another school does NOT qualify a student to retroactively apply Hazlewood at UT Health San Antonio

  • For the Veteran (Standard)

    • Eligibility criteria
    • Complete the Texas Hazlewood Act Exemption Application
    • Submit a copy of the Member 4 or Service 2 DD 214 Certificate of Release or Discharge from Active Duty (located in the bottom right-hand corner of the form):
      • Form must reflect that the Veteran Entered the military service from Texas, or Home of Record at the time of entry is Texas.
      • Form must indicate more than 180 days of federal military service (does NOT include Basic Training) and that the veteran received an Honorable Discharge or General Discharge under Honorable Conditions. If reservist, DD214 must show veteran was called to active duty by a branch of the US Armed Forces and served at least 181 days on active duty.
    • Certificate of Eligibility (COE) for federal GI Bill® benefits – to verify eligibility for the Veteran’s, or dependent’s federal education benefits, obtain a COE by completing an application at https://www.vets.gov/education/apply/
      • If the Veteran’s final discharge is prior to 9/11/2001 the certificate of eligibility is not required.
    • Valid Texas Driver’s License or Identification card
    • Submit a copy of your Hazlewood Online Database Registration
    • If concurrently enrolled at another school, veteran must provide completed Request for Record of Credit Hours” Form.

    Students requesting to renew benefits for subsequent and continuous enrollment will need to submit the Texas Hazlewood Act Exemption Application for Continued Enrollment

  • For the Children of the Veteran (Legacy)

    • Eligibility criteria
    • Complete the Texas Hazlewood Act Exemption Application
    • Submit a copy of the Member 4 or Service 2 DD 214 Certificate of Release or Discharge from Active Duty (located in the bottom right-hand corner of the form):
      • Form must reflect that the Veteran Entered the military service from Texas, or Home of Record at the time of entry is Texas.
      • Form must indicate more than 180 days of federal military service (does NOT include Basic Training) and that the veteran received an Honorable Discharge or General Discharge under Honorable Conditions. If reservist, DD214 must show veteran was called to active duty by a branch of the US Armed Forces and served at least 181 days on active duty.
    • Certificate of Eligibility (COE) for federal GI Bill® benefits – to verify eligibility for the Veteran’s, or dependent’s federal education benefits, obtain a COE by completing an application at https://www.vets.gov/education/apply/
      • If the Veteran’s final discharge is prior to 9/11/2001 the certificate of eligibility is not required.
    • Valid Texas Driver’s License or Identification card for both student and parent
    • Birth Certificate, Adoption Documentation, or IRS issued Tax Transcript from current or previous year indicating dependency of child, and identification of designee if applicable. To retrieve a tax transcript, visit http://www.irs.gov/Individuals/Get-Transcript.
    • Submit a copy of your Hazlewood Online Database Registration
    • Student’s proof of Selective Service registration (not required for females) or proof that you are exempt. Retrieve proof of registration at: http://www.sss.gov
    • If concurrently enrolled at another school, veteran must provide completed Request for Record of Credit Hours” Form.

    Students requesting to renew benefits for subsequent and continuous enrollment will need to submit the Texas Hazlewood Act Exemption Application for Continued Enrollment

    Students claiming eligibility based on being a dependent on the Veteran’s income tax return, will need to submit a copy of the Veteran’s Tax Transcript every year to confirm their continued eligibility.

  • For the Children and Spouse of the Deceased Veteran

    • Eligibility criteria
    • Complete the Texas Hazlewood Act Exemption Application
    • Letter from the Veterans Administration indicating the Veteran’s death was service-related or DD 1300 Report of Casualty if the Veteran is deceased.
    • Submit a copy of the Member 4 or Service 2 DD 214 Certificate of Release or Discharge from Active Duty (located in the bottom right-hand corner of the form):
      • Form must reflect that the Veteran Entered the military service from Texas, or Home of Record at the time of entry is Texas.
      • Form must indicate more than 180 days of federal military service (does NOT include Basic Training) and that the veteran received an Honorable Discharge or General Discharge under Honorable Conditions. If reservist, DD214 must show veteran was called to active duty by a branch of the US Armed Forces and served at least 181 days on active duty.
    • Certificate of Eligibility (COE) for federal GI Bill® benefits – to verify eligibility for the Veteran’s, or dependent’s federal education benefits, obtain a COE by completing an application at https://www.vets.gov/education/apply/
      • If the Veteran’s final discharge is prior to 9/11/2001 the certificate of eligibility is not required.
    • Death Certificate for deceased Veteran
    • Birth Certificate, Adoption Documentation, or IRS issued Tax Transcript from current or previous year indicating dependency of child, and identification of designee if applicable. To retrieve a tax transcript, visit http://www.irs.gov/Individuals/Get-Transcript.
    • Valid Texas Driver’s License or Identification card for student
    • Submit a copy of your Hazlewood Online Database Registration for both the student and the veteran.
    • Student’s proof of Selective Service registration (not required for females) or proof that you are exempt. Retrieve proof of registration at: http://www.sss.gov
    • If concurrently enrolled at another school, veteran must provide completed “Request for Record of Credit Hours” Form.

    Students requesting to renew benefits for subsequent and continuous enrollment will need to submit the Texas Hazlewood Act Exemption Application for Continued Enrollment

    Students claiming eligibility based on being a dependent on the Veteran’s income tax return, will need to submit a copy of the Veteran’s Tax Transcript every year to confirm their continued eligibility.

  • For the Children and Spouse of the Disabled Veteran

    • Eligibility criteria
    • Complete the Texas Hazlewood Act Exemption Application
    • Veterans Administration Rating Decision Letter indicating the Veteran is totally and permanently disabled.
    • Submit a copy of the Member 4 or Service 2 DD 214 Certificate of Release or Discharge from Active Duty (located in the bottom right-hand corner of the form):
      • Form must reflect that the Veteran Entered the military service from Texas, or Home of Record at the time of entry is Texas.
      • Form must indicate more than 180 days of federal military service (does NOT include Basic Training) and that the veteran received an Honorable Discharge or General Discharge under Honorable Conditions. If reservist, DD214 must show veteran was called to active duty by a branch of the US Armed Forces and served at least 181 days on active duty.
    • Certificate of Eligibility (COE) for federal GI Bill® benefits – to verify eligibility for the Veteran’s, or dependent’s federal education benefits, obtain a COE by completing an application at https://www.vets.gov/education/apply/
      • If the Veteran’s final discharge is prior to 9/11/2001 the certificate of eligibility is not required.

    • If concurrently enrolled at another school, veteran must provide completed Request for Record of Credit Hours” Form.

    Students claiming eligibility based on being a dependent on the Veteran’s income tax return, will need to submit a copy of the Veteran’s Tax Transcript every year to confirm their continued eligibility.

  • Stacking Hazlewood Benefits with VA Education Benefits:

    Those who qualify for the Hazlewood Exemption may use the exemption in conjunction with all versions of the GI Bill®  except the Chapter 33 – Post 9/11 GI Bill® at the 100% rate or the Chapter 31 – Vocational Rehabilitation program.

    Any student that qualifies for both the Ch 33 Post – 9/11 VA Education Benefit and the Hazlewood Exemption will not be able to use the Hazlewood Exemption towards any tuition paid for by the VA

    Any remaining tuition and fees that can be exempted will be pro-rated as Partial Hazlewood

    Students must apply for their Hazlewood Exemption and their VA Education Benefits separately
