Award Year 2023 – 2024 Forms
- Texas Firefighter Tuition Exemption Request – For use in term(s) fall 2023, spring and summer 2024 (includes the Selective Service and Child Support Statements required for state-funded assistance).
- Nonresident Teaching\Research Assistant Waiver Form (now closed for academic year 2023-2024) ***Waiver requests are submitted once a year certifying the student will be employed at least half time or greater by UT Health San Antonio in a Teaching Assistant (TA) or Research Assistant (RA) position. Start and end dates of the position cannot exceed a 12 month period and college work study students are not eligible to receive this waiver based solely on their employment as a student worker. Departments will need to submit proof of student’s appointment as a TA or RA out of the Human Capital Management (HCM) system.Waivers are applied for each semester that fall within the position start and end dates after confirming the student’s enrollment status is at least half time for the term the waiver will be applied. Students in the Graduate School of Biomedical Sciences must be enrolled full-time to qualify for the waiver in accordance with their policy.
- Child Support Statement – Self-certification form for students receiving state financial assistance, indicating they are not behind on child support payments, as required by Texas Family Code, Title 5, Section 231.006. This form must be submitted every academic year.
- Pell Grant Statement of Eligibility – Submit only after receiving a request for this document.
- Selective Service Statement – this statement must be submitted at least once during a student’s academic program in order to receive state-funded assistance.
- TEXAS Grant Statement of Eligibility – Incoming and continuing students must complete this form to be considered for a TEXAS grant. UT Health San Antonio only receives “renewal” funds for prior recipients and cannot offer initial awards. Click here for more details.
- Submit a Free Application for Federal Student Aid (FAFSA) online here. Review the steps after you submit your application here. Please carefully consider which award year you are applying for:
- 2023 – 2024 FAFSA applies to fall 2023, spring 2024, and summer 2024
- 2024 – 2025 FAFSA applies to fall 2024, spring 2025, and summer 2025
- IRS Data Retrieval Tool Instructions – The easiest way to report your income on the FAFSA is by using the IRS Data Retrieval Tool (IRS DRT) within the application.

- Verification of Non-Filing – To order an IRS-issued Verification of Non-Filing, click here to access the IRS Get Transcript Online website. Please note the the IRS now uses a two-step multi-factor authentication process that requires you to register before submitting an online transcript request. If you are not able to successfully register, you can still request it via mail using IRS Form 4506-T. Please do not submit this form unless requested.
- Texas Application for State Financial Aid 2023-2024
This form is used if you are a resident of the State of Texas and you do not meet the citizenship requirements for Federal student Aid. Please submit this form along with the TASFA verification worksheet, 2021 tax transcript, 2021 W2 form(s). If you are a dependent student we will need your parents’ income information too. Please contact our office if you are not sure of your dependency status.
- TASFA Verification Worksheet 2023-2024 – Complete and submit this form along with your 2023-2024 Texas Application for State Financial Aid (TASFA) if you do not meet the citizenship requirements for Federal Student Aid but are classified as a Texas Resident by the Office of the University Registrar.
